The Accelerators section of that zip does have a 'Hardtail TQM.bin' rom image, maybe that'll work given there was only a small number of accelerators from that brand? The Expansion_Boot_Roms.zip located in The Zone! on the EnglishAmigaBoard has all the below Roms, if it's there probably just easier to download that.

Most likely contact issues. I'd remove the megachip and clean the socket pins and megachip contacts with contact cleaner.
