I worked as a field service engineer, repairing PCs in schools.

Unfortunately, a batch of PSUs were sent out from the supplier, with the switch set to 110v rather than 240v, but a sticker saying 240v was put over the top of the switch.

I had to visit a school and replace something like 40 PSU's that had all gone pop because somebody didn't do proper QA checks.

Some months later, I had to replace a PSU in a single machine in a university, but as it turned out, one of these faulty PSU's was still in the parts system, and it went off as I plugged in power, nearly giving me a heart attack!

To my defence I'm saying that two of the four PSUs have been repaired by me, one acknowledged by The PSU Master as faulty from production and this one, ermm... yes, this is hard to getaway with. ;)

Could be worse. My old manager got fed up we didn't set up the test-dialer we've gotten from another office (in the US). So he plugged everything in and powered on. $25.000 down the drain since he forgot to check the voltage of the system.
