Yeah. I think I was confused when looking it the pictures. Those are the 5V, 12V, and GND right?

Leftmost 4 holes for 5V
Middle 6 holes for gnd.
Right 2 holes for 12V.
EDIT:
Oh think I see where confusion is now... that is not mains input on the front, I just used mains cable for the 5V and 12V wires I needed something high current and that was all I had to hand.

I can answer the rest when I get time / the PSU but yes, it does get hot when pulling 12V a lot. I think the issue was actually something close to that - when I had the PSU "out in the open", it was fine but as soon as I put the case back on, let it work for 15 minutes - bzzzz. And that was it. Some diode died and the whole PSU was out of order. Maybe it's not a serious issue and can be fixed easily but still - yours doesn't do it.

Because that's exactly the reason why I had to order Centuriontech's PSU in the first place -- it has a bit more space on the right so I can use SD card adaptors like on the picture. In the old version, there is a couple of components in the way.Re: Future PSU designs

Never seen a IDE mash up like that. Its over hanging the PSU, so it's going to be in the way.

It's quite popular among Amiga people who for some reason prefer to have IDE female connectors angled to the left (contrary to the right what we would prefer). From your picture above it looked like the PCB is empty in that area except that black thing but maybe it's still too much.
