The only possible modification is wiring a 68R resistor in series with ROM CE to the dualtos board as I have seen issues there recently.

You probably would have to send me your motherboard to diagnose the issue because it's not a fault I have seen before. There's no difference really between 16MHz TOS104 or TOS206.
Can you please clarify the suggested resistor connection on the PCB?! (image)

The wire which goes to the ROM board, place 68R resistor in series with that wire.

OK, so need to see the 16MHz waveform output from the shifter pin 39, to pin 1 on the GAL top right of the booster PCB.

Also, what floppy drive are you using exactly ? and have you tried a different one ?

Also when it bombs, what happens if you press reset on the back of the ST, does it bomb again ?
